Consider Serial Notching Over Midline Suturing for Platysma Bands 11/3/2002

Serial notching of the platysma bands is a good alternative to face-lifts for some patients, Dr. Ziya Saylan said at a dermatology symposium sponsored by Mount Sinai School of Medicine.

Serial notching can be more effective and cause fewer complications than midline suturing, which requires revision and scar correction in about 14% of cases, said Dr. Saylan, a cosmetic surgeon in Düsseldorf , Germany , and chairman of the European Academy of Cosmetic Surgery.

Other advantages include the use of local anesthesia, a quick recovery period, short duration of surgery, and no large submental incisions for platysmal band plication. Serial notching can be performed alone or in conjunction with a lower face-lift, posterior neck lift, or submental lipoplasty.

Incisions 1-1.5 cm long are made 3-4 cm apart. The muscle band is undermined, dissected on both sides, and extracted using a small mosquito clamp, Dr. Saylan said.

Notching is performed via electrocautery. Suturing is necessary only if the skin is quite lax—otherwise, wound-closure tape can be employed.

Use of compression bandages for 2 days is recommended. After 5-6 days, the platysma bands are no longer visible, even when contracted.

Of 51 patients who underwent serial notching in a 14-month period, all were satisfied with the results, Dr. Saylan said. No major complications occurred.

Thirty-nine patients had an S-lift or face-lift in addition to the serial notching and observed better results.

Submental lipoplasty was also necessary in 40 patients (Aesthetic Surg. J. 21[5]:412-17, 2001). The average patient age was 61 years.
